Rhinoplasty

Rhinoplasty also known as a Nose Job is a surgery aiming at correcting the shape of the nose. Rhinoplasty is the most frequently performed plastic surgery. Rhinoplasty can expand or reduce the size of the nose. The tip of the nose can be modified. If there’s a bridge in the nose, it can be corrected. If there’s a problem in breathing, it can be fixed with rhinoplasty. The change in the shape of nose can change the appearance of the face completely and make it better.The most important point is that after the operation, the nose should look natural. *Rhinoplasty can be performed after 18. It is not advisable to have it before as the bone development is not complete. There is no upper age limit for this operation. If the patient’s general condition is good, it can be done in elder people.

WHO NEEDS RHINOPLASTY?

• Patients who have problems about the shape of their nose inborn or after some trauma

• Local Anesthesia
• General Anesthesia (It is preferable for the comfort of the patient and the doctor)

• Prior to the operation, for 10 days aspirin and blood thinners shouldn’t be used.
• One week before the operation, smoking should be stopped, as it may increase the swelling and bleeding during and after the operation.

HOW IS IT PERFORMED?

1.Closed Rhinoplasty: In a closed rhinoplasty treatment, cuts are done through nostrils, so there’s no cut and scars. In the appearances auto-disdoing stitches are put inside the nose and those stitches are not removed. 2.Open Rhinoplasty: In an open rhinoplasty method an incision is made to the skin between the two nostrils (columella) and the skin is lifted. Then the surgery is performed and the incision is stitched. Due to this, only a small scar is left on the skin between the nostrils, and it disappears in 1-2 months. *The choice of nose surgery method in Turkey depends on the shape of the nose of the patient and the method the doctor prefers. In rhinoplasty surgery, the bones forming the nose are broken and/or rubbed, cartilages are cut, removed, thinned and sometimes added. For medical intervention to the nose, tools such as osteotom and rasp are used. If necessary, cartilages can be added to certain parts of the nose. These cartilages can be collected from the septum, ears or ribs. In such cases, the patient is notified before the surgery.

• Ultrasonic Rhinoplasty (Piezo Rhinoplasty): Ultrasonic technique (Piezo technique) is used. That is by piezoelectricity ultrasonic surgery equipment, bone and hard cartilage tissue are cut through ultrasonic waves. In piezo rhinoplasty method, the ultrasonic waves just affects the bone and hard cartilage tissue, it doesn’t harm soft tissues like mucous membranes, vessels and nerves. By Ultrasonic rhinoplasty surgery, a better controlled, proper cuts and shapes can be obtained. For this reason, esthetic success increases, and after ultrasonic rhinoplasty surgery bruises, swelling and redness are less visible. Ultrasonic surgery (Piezo surgery) is generally used in open rhinoplasty method, it is more difficult in close rhinoplasty as the access of instruments to the targeted area is not easy.
• Non-Surgical Rhinoplasty: This method allows us to correct some imperfections on the nose using filling materials, suspension sutures and laser. In the last days liquid rhinoplasty method is very popular among patients. Through this method, droopy nasal tips can be lifted, low or bumpy bridges, asymmetrical nostrils and small defects resulting from a nasal surgery can all be corrected. Nose-reduction is not possible in this method but requires a surgical procedure. Non-surgical nose job is not a permanent solution. The nose may regain its previous shape so the procedure needs to be repeated after a given time. Some patients particularly prefer a reversible nose job. Sometimes, a combination of non-surgical and surgical nose job methods and use of local anesthesia may yield very satisfying results.
• Video-Endoscopic Rhinoplasty: The operation is performed based on the intranasal image taken by a 1-2 mm diameter camera and projected on a screen.
• Nasal Tip-Plasty: It is the correction of the nose tip. Sometimes the only problem is with the nose tip which can get corrected via surgery without any need to intervene in the other parts.

After the nose job surgery, generally silicone splint buffer are put inside the nose and removed in 2-5 days. During the removal process, generally no pain and bleeding occurs. After the operation, a flaster is put on the nose, and on top of it, a metal or thermal splint or plaster cast is made. After the rhinoplasty surgery, there may be some edemas or bruises under the eyes and face. For this reason, after the rhinoplasty operation, cold compress on the cheeks and under the eyes is advisable.

• The atel on the nose is removed after one week.
• Flaster is removed after 2 weeks. (The patient can remove it after 2 weeks)
• If open rhinoplasty surgery is performed, the stitches on the tip of the nose are removed together with the atel in one week.
• Swelling and bruises generally disappear in 2 weeks.
• The patient can continue his/her school and work after one week.
• Some patients are advised to message on their noses. It can be continued for 3 months.
• After the nose reshaping surgery generally no pain is felt, if necessary some antibiotics and pain-illers can be used.
• The nose shouldn’t be touched with water.
• If yo sneeze, you should do it by breathing through the mouth without pressure.
• You shouldn’t clean or blow your nose for one week after the nose surgery.
• You shouldn’t smoke for 2 weeks after rhinoplasty.
• You shouldn’t drink alcohol for 2 weeks.
• You should beware of heavy physical activities. You should protect your nose from sudden strikes at least 1 month. You can perform your daily activities but you shouldn’t lift heavy things and you shouldn’t bend.
• Especially first 24 hours you should put your head higher than your body.
• No drugs should be used without informing the doctor.
• Glasses shouldn’t be used for 2 months.
• You should protect yourself from direct sun rays for 2 months. You can use a hat protecting your nose from direct sunlight in the sun.
• After rhinoplasty, people can make different comments. This is because people have different tasted and thoughts. It takes approximately one year to have the latest shape of your nose.
• If revision rhinoplasty is required, you should wait for one year. (In some special cases it can be done in 6 months)
